The Independent Personal Services Form must be attached to the Honorarium or Expense voucher when making payments for services of $600.00 or less per voucher or $2,000.00 or less per year. This form must be attached for payments of any amount to speakers and entertainers.

Payments for larger amounts will require the attachment of a Professional Services Contractual Agreement form instead of the Independent Personal Services form except for payments to speakers and entertainers.

Payments to non-resident aliens will require the attachment of the IRS Form 8233.
